Parents are reminded that Andover Public Schools requires a Criminal Offender Record Information (CORI) check on anyone who volunteers at a school or school event. This includes chaperones for field trips and visitors to classrooms for academic or social functions. (More information on the Massachusetts CORI program can be found on this web page.)
Andover’s CORI checks are valid for only one year. If you would like to volunteer during the 2015/16 year, please fill out this APS CORI check form and bring it to the Bancroft office along with a photo ID, even if you have done this in the past.
The form needs to be returned in person to the Bancroft office, where a school administrator will need to see an original government issued ID (drivers license, passport, etc.). No ID photocopies can be accepted, and it must be presented by the person submitting the CORI form.
CORI forms will be accepted live at Bancroft between 9am and 1pm during the summer, and during school office hours once school opens in September. If you have any questions, you can call the Bancroft office at 978-623-8880.
